If you have been selected by the Kansas Department of Wildlife & Parks to participate in any survey you would have received a postcard directing you to a specific website.

INSTRUCTIONS TO USE

If you received a card

• To participate in the survey you need to go to the address that is listed on the postcard (each survey has a different address).
• At that address enter your identification number exactly as it appears on the front of the postcard. Only responses associated with valid identification numbers will be tabulated.

The department appreciates your cooperation in completing any survey.
